For someone with diabetes, many factors contribute to keeping blood sugar levels even and within the normal range. Diet, exercise, medications, and seeing your doctor regularly all play an important role.

Do you have a written plan to manage all the different aspects of your diabetes? If so, does it include all the different items you are using, or just some of them?

What kinds of things do you monitor to ensure your plan is working well? Do you monitor your blood pressure? Weight? Blood sugar levels? Anything else?

If you do keep a written plan and record, have you found it helps you? How so?
